Q: What is needed to place an order for fabrication?

1. A fully completed fabrication work order form provided by Advanced O&P Solutions. This shall include all measurements, materials and special instructions.
2. Patient’s name or number that you have assigned. This will allow us to reference during any communication.
3. Any and all tracings and X-rays
4. A negative mold, positive unmodified mold, positive modified mold or proper measurement chart for digitizing.
5. Please put a purchase order number on every work order.

Q: What is Advanced O&P Solutions warranty on fabrication?
A: Definitive devices fabricated by Advanced O&P Solutions have a 90-day warranty from the date of shipment from our facility. The warranty covers defects in materials and workmanship only. Test sockets and other partial assemblies are not covered under this warranty and this warranty does not cover damage due to accidents, misuse, tampering, alteration or improper use.

Q: Occasionally I would like to speak to a fellow practitioner regarding fabrication needs, does Advanced O&P Solutions have Certified Practitioners on staff ?
A: Advanced O&P Solutions has both an American Board Certified Orthotist and Prosthetist on staff.

Q: Can Advanced O&P Solutions provide components for my fabricated devices?
A: Advanced O&P Solutions can provide components. We stock many of the components you use. If we do not stock a particular component we will be happy to order it for you.
