Net 30 days from date of invoice. We accept the following charge cards for immediate payment: Visa and MasterCard

Past Due Accounts

We reserve the right to suspend shipments if any portion of an account becomes past due. Past due accounts will be put on credit hold unless satisfactory arrangements have been made with the credit department.

Service Charge

Past due amounts are subject to 1-1/2% per month service charge, effective annual rate 18%.

Shipping

Qualifying Labs within the Greater Chicagoland area will be shipped via Advanced O&P Solutions Transport. No shipping charges will be assessed when utilizing Daily scheduled AM Delivery to and from your facility. Additional Pickup and or delivery will be assessed an expediting fee to the specific work order at 20% or 10% of cost of fabrication.

All non –qualifying labs will be shipped via UPS Ground. Next Day and 2 nd Day shipping charges will be added to your invoice.

Submission Guidelines for Fabrication

The success of each individual patient fitting depends on the accuracy of the information you provide. It is our goal and mission to provide your patients with quality prosthesis and orthosis. This will be achieved by working closely with your team and to always provide and maintain open communication. Please submit the following information with each order:

1. A fully completed Fabrication Work Order form provided by Advanced O& P Solutions. This shall include all measurements, materials and special instructions.
2. Patients name or number that you have assigned. This will allow us a reference during any communication.
3. Any and all tracings and X-rays.
4. A negative mold, positive unmodified mold, positive modified mold or proper measurement chart for Digitizing.
5. Please submit all Fabrication Work Orders Via Fax the evening prior to pick up day. Fax # 708-598-2857.
